You're right ncmusician_7. Two of the more subtle ways are appearance fatigue - though that kicks in after a pitcher has appeared, I think, in two-thirds of a team's total games, and the exceeding of a pitcher's RL IP/G. This latter needs to be taken into account when setting pitch counts, as a pitcher who might be used in fewer games than in RL, say because they pitched in a 3 or 4-man rotation will now pitch in a 5 man rotation, nevertheless suffers fatigue if their settings go well over the IP/G they had in RL, say a pitcher with 4.5 IP/G being used for 7 IP a game will still run into problems.
