Carlton for Wise is a close second.... tho Wise was the de facto ace of the Cardinals staff until traded for Reggie Smith.   St Louis would have been able to compete with the Big Red Machine in the mid 70s had Carlton not been traded away.   The Cardinal organization gets a pass as one that rarely makes mistakes but I also offer the Mulder for Haren deal as an honorable mention here.  I am willing to say the Cardinals would have another WS championship or two had that trade not been made.  I am friends with a guy who had input on that trade and he told Walt not to make the deal that Haren was one of the top competitors he had ever seen toe the rubber... a three time all star.

Dennis Eckersley for 3 minor leagues. None reached the majors
Sparky Lyle for Danny Cater

Mulder was coming off of 4 outstanding seasons in Oakland. Haren was still considered a prospect. Mulder went 16-8 in STL before shoulder issues stalled his career. Mulder appeared to be the better pitcher at the time of the trade...hard to see how Walt gets slammed on this one..

John Smoltz for Doyle Alexander wasn't that bad. Every GM will tell you that they would make that trade anyday. The Tigers were in a playoff race and needed a pitcher to put them over the hump. So they traded a seemingly low potential prospect for Doyle Alexander. Doyle had a 1.80 ERA down the stretch for the Tigers. Just so happens that that one low potential pitcher is a hall of famer

The Cubs-A's Eckersley trade also doesn't belong in the same category, not even close.  Eck was 32 in 1987, had been either ineffective, or hurt, or both, for about 6-7 seasons.  In 1986 he was 6-11 with a 4.57 ERA in 33 starts.  No one - absolutely no one - thought the Cubs were getting raked in that trade at the time.

As it turns out, Eck was about to go on a 5-year run that ranks with the best of any relief pitcher in history, but no one foresaw that in April of '87, including the A's....
