My guess is that with injuries "on" there is always a chance that a fatigued player will get injured.  It should be a pretty small chance for a SP at 90%, but then again, it should near impossible for Cecil Fielder to hit an inside the park HR in the SIM... but I've seen it happen.

You took what should have been a relatively safe gamble, but lost.  Happens to the best of us.
