I have a LF with a 45 Glove who made four errors on fly balls in one game. That is more errors than he made in the entire previous season. Is this an indication that he is tired or has an injury?

If he's at 100% he is not tired. If he's injured, he won't play. So no. It's a sign that he's a weak defender and random stuff happens all the time in baseball.

You have to take the good with the bad with a LF with a below 50 rating. Hope he has a good bat.
