Is the face to face contact just with the head coach? As far as a recruit being able to sign with you.

The minimum is Scouting Trip (Assist. Coach) for them to sign with you.

Head Coach Scouting Visit
Assistant Coach Home Visit
Head Coach Home Visit
Campus Visit

It was a long time ago, but had a recruit green with only a assistant coach scout and did't sign. I don't know if they changed anything since then.

Nope. You can get a recruit green without a Face to Face. It is rare but it does happen.
But he still won't sign with you without the Face to Face.

Players that are scouted during the season will sometimes show up as green without any other effort , also.

According to what I know that shouldn't have happened - Unless - Another coach came in and Scooped him from you on the last round.
That's definitely possible since you only had an assist coach scout on the guy.
