It's rare for a fielder who fields 1.000 to make errors but it IS possible. It's especially possible if he ever plays below 100%. Fielding while fatigued is a much faster drop off than hitting while fatigued.

It also depends on the number of games played as I recall there are fielding penalties for < 20 games < 10 games and < 5 games at a position.

Assuming that's Elston Howard, it doesn't appear he has appeared in a game fatigued, so it's not for that reason. I believe it's possible for any player, regardless of perfect fielding percentage, to get a throwing error. It would work similarly to a pitcher with a HR/9 of 0.00. There's a baseline minimum that allows for a small chance of that event happening anyway.
