FINAL UPATE - another example of customer service steadfastly refusing to investigate a more-or-less definitive bug. Oh well. I tried. If this happens to anyone else, feel free to let them know "immediately" (like I did in this case), so seble can be notified.

This code has been in place for years, so I'm not sure there is a bug involved. We would need to be alerted immediately to be able to verify. If it happens again please submit a ticket right away and indicate that it's urgent and should be directed to seble immediately. Thanks.

I worked in IT for years, and "it's been that way for years" was one of our forbidden phrases.  Bugs can get into your code early and not be discovered for years, so it's never too late to learn about it.

That said, I had this happen to me a couple of times in both D3 and D2 - not often, but enough to make me gun shy about sending HVs/CVs before I contacted the kid.  I always figured it was just part of the system.  Next time it happens I'll be sure to submit a ticket.
