It is definitely not the case that a Set Up A (or Long A) is always used before Set Up B (or Long B). Different situations lead to different usage patterns. crazystengel's suggestions are the best way to go. ozomatli's suggestions are also good.
In your particular game, O'Brien got pulled with the score 3-0 in the top of the second. It is likely that Sparky thinks that's not a good time to burn a Set Up A...whom he will regard as one of your best pitchers...in this case, you gave him lots of other options.
Note that, as far as I know, Sparky does NOT consider the importance of the game to the overall standings. This would be very difficult to program, I am sure. So if you really want him to make certain decisions, you have to basically give him as few other options as possible.
Other things to consider....
-- Uncheck the PH box for the pitcher in the advanced sittings. Even if you set call bullpen = 1, if there is a situation where Sparky thinks a PH is warranted, and he has other pitchers available who can come into the game, you may get PH for. This is risky and can backfire...notice that in your game, the reason Wood got pulled is you were trailing in the bottom of the 8th and Sparky sent up a pinch hitter....who promptly singled in the tying run with 2 outs.
-- Increase Wood's TPC and MPC to max levels. Yes, there is a small likelihood that the game goes deep into extra innings and he ends up pitching fatigued....but I would rather take that chance than risk having pulled while he has ANYTHING left in the tank
