my practice plan formula, for the most part:
green-15
blue-12
black-10
orange-5-8
red-4-6
I change it once during the season about midway, maybe giving more to green as other categories turn colors.
And I agree with this for the most part. Except for the red.

I also change my minutes much more often. If I happen to notice something glaring, I change it immediately. If I have a sophomore or junior that has basically all black categories, if I see one change to orange, I'll drop it to 8 immediately. There's times where I may have 15 minutes on a black category. And I feel like if you don't change it more often, that's wasting a TON of minutes.

You said you change it once during the season, if you have 15 minutes in a category that changes orange after one day, you're missing out on 7 extra minutes or so, for a half a season. That can be a big difference.

I put 0 on most reds if I need the minutes elsewhere at the beginning of the season. If during the season it drops I will move it to 7 if it is an essential category for that player. Red FT is usually kept at 7 just because you won't see a numerical change.

Yellows are usually 9.

Never put more than 30 minutes of practice in any category.

I have a 2 WE player who increased his PER score this season after one game, despite zero minutes of practice time in PER. I never had any ratings increase with zero PT before - was that possible before, or is this another example of one of the random weird bugs that have suddenly popped up?
Did it go down in the off-season? Sometimes players gain back some of what they lost in the offseason after the first practice.
Yes, it had gone down in the off-season. That would explain it. I didn't realize that was something that could happen. Thanks for the info, Trentonjoe.
