IIRC, If you meet the initial demand and they come back with an "on second thought" offer that you do not/can not meet, you will not receive a Type-D. It has to be a total rejection of playing pro baseball for staying an amateur (or going pro in a different sport).
That is incorrect. They just have to reject an offer that meets their initial demand. You do not have to make an offer on their subsequent demand.
As to the question posted by the person who started this thread, I can only speculate that you either did not meet their initial demand, or that you withdrew your offer before it was rejected.
