I think 1st-3rd round picks that refuse give you Type D compensation. Do the sandwich picks at the end of the first round also get Type D picks?

No, they do not.

where do the unsigned players that generated the Type D picks go? Off to college, back to college (otherwise back in the draft the following year) or do they become regular FA during the offseason to be signed and undrafted FA the following season?

If they were college seniors, they show up as minor league FA's the following season.  Everybody else just disappears, never to be seen again.

Not exactly true.  If a college senior is unsigned who was threatening to do something else (eg, play pro basketball despite being 5'10" and having speed = 37), he does that something and does not show up as a minor league FA.  Not sure if this is all seniors that are not "looking to sign," or just those that specifically state an alternative plan.

Just noticed this a season or two ago.
