The form is off the knowledge base. I always get there by opening up a box score and clicking the dots in the upper right corner next to the clipboard icon and choosing "Support" -- it'll open the KB page up and then you just need to click "Submit a Ticket" from the top list of options.

the way it is set up in the 3.0 redesign is not at all welcoming for contacts with support - it wasnt great before - now it is darn hard
