I admit this is a small sample size of 1 ML team......

In 2022 at the ML level the World Series Champion Houston Astros placed 11 different players on the IL (disabled list), 10 during the regular season and 1 in the postseason. 2 of those 11 were season ending (out for season) injuries.

For those who are frustrated about injuries to star players how about this list: Altuve, Pena, Verlander, Presley and Alvarez. That is some serious star power the Astros had on the IL during the season although all of them were healthy for the playoffs. That list doesn't include Brantley who was 1 of the 2 out for season injuries.

In this game I don't think I have ever had 11 different players go on the DL at the ML level in one season. Injuries are not really out of hand in this game compared to real life.

I've played 64 HBD seasons now. I've had ONE player have his entire career wrecked by injury - a first-round, 9th overall SP with 80+ Health who had not one but two elbow surgeries.
Even for always rolling out 20M Medical, that ain't bad.
