Hill hurt his hammy in S20. Range, speed and baserunning took a hit. That 9m medical didn't help.
I think the interesting thing is the continued decline. I wouldn't think the game is programmed for lingering injuries although, if you've ever pulled a hammy, you know the damn thing never gets back to 100% unless you stop for an extended period of time.
I also wouldn't think that the game is programmed for lingering injuries, but my former player
Marshall Whitney makes me wonder, when you look at his DL history:

Three labrum tears within two seasons seems a little odd, doesn't it?
The other funny thing about him is that he was coming off of two pretty decent seasons in S11 and S12 when he suffered the first labrum tear right at the end of the S12 regular season. Took a ratings hit, but recovered over the off-season. At the beginning of S13, he was absolutely horrible out of the gate until the labrum tore again. It was almost as if the problem was there and he was (unsuccessfully) trying to pitch through it when it blew up. Went on the 15-day DL, came off, had a few more unsuccessful starts when it blew up for a third time.
