I accept them all.  I figure if someone takes the time to send me a request, then the right thing to do is to accept it if I can....

If I don't accept a game, I often sitemail the coach and explain why, so that he knows I'm not ignoring him.

Schedule requests that get sent out during recruiting are especially likely to not get noticed, unless one is specifically looking for them... a lot of people are focused on recruiting, not scheduling, at that time, and those requests are bound to get lost among literally dozens of HV/CV messages.
I always send requests during recruiting.   Figuring, as professor said, that my request could get overlooked,  I cancel and re-offer after recruiting is done.This puts my request at the top of the coach's box

i tend not to reject so the same coach doesn't - accidentally or on purpose - try to rechallenge me.
