I've opened a ticket about this but haven't gotten a good response yet.  Does anybody know how the player rest vs. defensive replacements works?

My settings were set to:

use player rest if winning by 5 or more in the 8th or later
use defensive replacements if winning by 1 or more in the 8th or later

I won a game 27-2 and my player rest did not kick in.  I opened a ticket and they responded that it was because defensive replacements always over-ride player rest, and with my current settings player rest would never kick in.  So I changed my player rest to if winning by 5 or more in the 7th inning or later.  Now I play a game were I am winning 9-1 in the 7th and nobody get rested.

My rest and defensive replacement heirarchies are the same and filled out (down to 5th spot on every position).

Anybody have any insight into why my players aren't being rested?

If, for example you starting shortstop, Joe Smith, is listed as number one on your rest heirarchy, and Kip Tuckerson is listed as number 2. Kip Tuckerson isn't going rest Joe Smith, you'd want to reverse them.
