If you have a guy who is a senior that didn't play one minute his freshman year (no redshirt or anything, just never got into a game), will he come back for his 5th year or does that Freshman year of no minutes still count for eligibility?

Still counts - only way he'd get a 5th year is through a RS (officially applied) or an ineligible season (and he has to meet the GPA requirements in D1/D2 to come back in those cases).
