Neither were injured nor tired (they weren't on the floor to finish first half). Is it because they had 3 fouls? If so, has this always been the case? Also, is there some way to change that?

Yup, unless you set them to "more aggressive" when they have 3 fouls at halftime they will not start in the 2nd half.
