I just noticed that 1993 Mike LaValliere's Chicago WhiteSox split-season caught stealing seems to be way off.  He was actually caught 23 of  32 would-be-basestealers for a 73% caught stealing percentage.  The sim database has him at 0 for 0  or 44% .

I heave heard of the error with Bert Campaneris who has an A+ arm even though he was 0/1 at catching base stealers.  But does anyone have an idea how common these "errors" are?

I think if they don't meet some minimum # of innings (games, attempts... something) then the sim estimates their CS%

It's strange to see an error when there is actual, reasonable data though.
