At what fatigue percentage do injuries start to occur? I've never really looked at fatigue too much or tested its limits. What kind of insight or experience do you have?

I generally start to see it after they drop below 90%. But I think they can occur at any time.

I've seen one at 100% due to in-game fatigue.... the lower the fatigue, the more common, but they only occur on a play, so less common for hitters than pitchers even at same fatigue levels due to distribution of plays...

Here is an example from the current cycle of an injury to a 100% player due to in-game fatigue.

For when the link stops working:
2007 Ron Villone had a RL IP/G of 1.14. In this game he threw 6.1 IP throwing 254 pitches. He was at 100%. This was the 2nd game of the season and his 1st game of the season. There was no prior fatigue or pitches thrown by Villone in this season. Here is the 8th inning:
8
8
1 Inning (At A Time)
W.Schang enters the game as a pinch-hitter for A.Fowler.
W.Schang lines a single to CF.
DP B.Joyce takes ball four.
DP K.Kelly flies out to LF. W.Schang is safe at 3B in a close play.
R.Villone was injured on the previous play.
K.Igawa enters the game to pitch.
DP C.Seymour is frozen on strike 3.
G.Cravath walks.
J.Posada can't handle the pitch and is charged with a passed ball. All runners advance. W.Schang scores.
M.Donlin takes ball four.
K.Marte flies out to deep LCF.

That's interesting, just4me. Do you suppose Villone really got "injured" in the usual way, or was it a case of him hitting the 255 pitch count?

Yeah, I think it was a genuine injury. I think Villone pushing the 255 threshold was just coincidence. This could’ve happened 20 pitches earlier or 20 pitches later. On that note, I’ve had several pitchers exceed the 255 pitch cap. In the box score it will show 255 pitches, but in their game log it will have 308 pitches or however many they actually threw above the 255 display limit.
