Why does it do this, when my settings are normal and I have a bench full of rested pinch-hit options? Seems to happen at least once or twice a game, and it's killing my offense.

If you want guys to PH more frequently, bump up that general PH setting to a 5, and then make sure your PH hierarchy is set up with the guys you want to come in available towards the top of the list.

I PH very aggressively with tight PCs, lots of available hitters, etc... but that general setting and the hierarchy are the two most important.

You'd probably want to go into the advanced settings and mark a lot of position players as unavailable for being replaced by a pinch hitter, before moving the PH setting to 5. Otherwise, you can have decent hitters removed from the lineup, and by the time the pitcher's spot is up late in the game, you might have one pitcher pinch hitting for another.

Absolutely do what dannyjoe said, otherwise you'll have guys being PH for you definitely don't want being PH for.
