Does that apply only to + Plays or is a OF with A+ Range going to cut down on XBH anyway and it is not necessarily shown as a + Play ... Also then if the OFer was a D/A+ he could reach the Ball and misplay it or not Catch as an error ?

it shows as a + play when the outfielder turns a double into a single or a triple into a double.

in the sim outfielders only make errors on dropped fly balls. sometimes they will get to a flyball using their great range then drop it and it shows up as +- in the play by play, but the more common type of errors outfielders make in real baseball (errant throws or bobbling a ball to allow a runner to get an extra base) don't happen in the sim.

D/A+ OF will cut down on XBH, yes, but they will show up as a + play.

Sure you'll see something like this in the PbP

Yes all the time. Majority of + plays for outfielders will be hits turned to outs in my experience
