I put a ticket in this several years ago as a bug, but neither I nor Tom/Paul could figure it out. When I came back recently, it happened again and I put a new ticket in on it. We still couldn't figure it out. I spent some time trying to isolate what caused/allowed this particular bug to happen and after isolating it and demonstrating repeatability, I gave all of the info to admin. It is on their list of bugs to fix, though I asked them if we could just leave as-is or roll back the 40 PC minimum for starters given the growing prevalence of openers and bullpen games in MLB. They said it is very low-priority and won't be fixed any time soon.
I've been asked about it in every league I've tested it and used it, and I told admin I'd share in the forums so it would be fair to everyone.
You can set your starters (any role) to any pitch count you want, including a 5/5, 5/15, 25-35, etc... so if you want to start Mariano Rivera, Sean Doolittle, and Eric Gagne in a 3-man rotation or even a 2-man rotation you now can without having to worry about in-game fatigue.
To set it up, you set the pitchers you want as starter as relievers (any role) with the PC settings you want them to have as starters. LEAVE YOUR ADVANCED SETTINGS WINDOW OPEN after you save those settings. Switch over to your managers center window and move them to the starter roles you want them in, switch back to the advanced settings window you left open (do not open a new one), and re-save without changing any settings for them (you can change the settings for the guys you are not moving to starter). They will now display as "none/none" however, the previous settings will hold despite this visual change. If you set them to 5/15, it will still be 5/15, even though it says none/none. DO NOT SAVE YOUR ADVANCED SETTINGS AGAIN or else it will save it as none/none and you'll find your 5/15 starter throwing 117 pitches and being fatigued for the rest of the season. If you need to make changes to any settings (maybe you called up a AAA guy), you need to redo the whole process of moving them to the bullpen and re-setting/saving their PC and moving them back to their starter roles with the advanced setting window left open.

Silver King I have that has started 80 of 80 team games, is SP1, and has not thrown more than 10 pitches in a game (more than 7 only 4 times). (This is also a good example of the
appearance fatigue I just posted about elsewhere in practice.)
